Job Overview

Revex Square LLC is seeking a dedicated and qualified Physician Assistant to support US-based supervising physicians in delivering high-quality patient care through Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.

The ideal candidate will assist with patient coordination, clinical documentation, remote monitoring, follow-up calls, and care plan implementation under physician supervision, while ensuring compliance with applicable US healthcare regulations and standards.

Key Responsibilities

Assist supervising physicians with patient scheduling and appointment coordination.

Review patient medical records and clinical documentation.

Support the development and implementation of individualized patient care plans under physician supervision.

Monitor remotely transmitted patient health data and promptly report significant clinical findings to supervising physicians.

Conduct patient follow-up calls and coordinate care for RPM and CCM services.

Document patient interactions, assessments, and care coordination activities in EMR/EHR systems.

Educate patients on chronic disease management, medication adherence, and the proper use of remote monitoring devices.

Coordinate with US-based physicians and other healthcare professionals to ensure continuity and quality of patient care.

Maintain compliance with HIPAA, US healthcare regulations, and organizational policies.
