Description

DataQ Healthcare is Looking for qualified professionals for the role of Diabetes Educator (Care Facilitator) who are passionate about remotely delivering care to the elderly and educating clinical staff and care coordinators. The ideal candidate should have an interest in learning new methodologies in team development and compliance adherence.

EDUCATION:

• Certified Diabetes Educator (required)

• Bachelors Degree in Health Care

• RN License (preferred)

Experience: Fresh to year as diabetes educator or in health care area.

LOCATION: Shahrah e Faisal, Karachi

Time: am – pm (from office) and half to hour in a night meeting (from home). Work days are from Monday till Friday.

DUTIES:

The candidate is expected but not limited to perform the following tasks:

• Diabetes Educator educates patients on the necessary lifestyle changes involved in a diabetes diagnosis; including nutrition, medication, physical activity and glucose testing.

• Collects patient data to monitor health status and proactively manage the patient's condition.

• Collaborate in developing a curriculum for clinical coordinators to deliver protocol-driven care (basic material will be provided).

• Gathering the most important information from the provided material or other peer-reviewed resources and formulate presentation material like PowerPoint slides on a provided background template.

• Deliver online recorded and live course materials

• Responsible for monitoring and acting upon patient's chronic care management parameters.

• Responding to any concerns from the care coordinator
